Brisbane Girls Grammar School has scheduled immunisations for Year 7 students on Tuesday 8 March. On this date, students will be able to receive their first dose of Human Papillomavirus (HPV) and the Diptheria, Tetanus, and Pertussis (Whooping Cough) immunisation.
Your daughter has been provided with an immunisation consent form to complete. If you consent to your daughter receiving her immunisations, you will need to sign the immunisation consent form.
We would appreciate the immunisation form being completed and returned to the Health Centre by Friday 25 February, in order to ensure your daughter’s immunisation can proceed on 8 March.
